Incident report template for a website outage

A short incident report for a client. Fill the blanks, copy the preview, or download the Markdown. The example under it uses the sample digest, and it does not invent a cause.

Filled from the sample digest

Harbor & Co is the public sample. It is not a customer. The digest for the period ending 29 September 2026 says the homepage, TLS, and domain were healthy, with one short miss on Thursday that recovered the same morning. Uptime: reached on 332 of 334 checks, 99.4%. I am not adding a start time or a root cause the page does not have.

Incident report

Client: Harbor & Co
Site: https://harborandco.example
Started: Not recorded as a clock time. The sample digest marks 24 September 2026 as the day with a miss.
Noticed: On the digest for the period ending 29 September 2026.
Recovered: The summary says the miss recovered the same morning. No clock time is stored.

What a visitor saw
The homepage missed checks. The digest does not say what the visitor read on screen. Leave that blank unless you saw it.

What we did
The sample work log does not invent a root cause. Do not add one here to make the report look finished.

What we will change
None recorded on the sample.

Record
Homepage checks: Reached on 332 of 334 checks (99.4%). Two checks missed.
Report: https://retainerproof.com/demo
Status page: This sample has no public status page. A live status page would list probe incidents for 90 days. It would not add a cause the checks do not know.

This is the public sample digest. Harbor & Co is not a customer, and the numbers are sample data.

Download the example Markdown. Open the live sample digest beside it. The emails are the notes you send while it is happening. This report is the page you keep after. A dollar figure, when you want one, comes from how to calculate downtime cost on their numbers.

Questions

Is this a postmortem?

Only if you know the cause. The template has a line for what you will change, and it allows none. A missed homepage check is not, by itself, a root cause.

Does the status page write this report?

No. The status page shows current state and probe incidents from the last 90 days. It does not write the visitor impact or the follow-up. You do.

Do I need an account to copy this?

No. The preview and the Markdown stay on this page. Nothing is stored.

Plans are Starter at $9/month for up to 5 sites, Studio at $29/month for up to 25 sites, and Agency at $79/month for up to 100 sites. Studio is the plan marked recommended. Starter, including a trial, keeps a small Powered by RetainerProof link on the client digest and the public status page. Studio and Agency remove that link. Prices are on the pricing page.
